Immunization Schedule

The following is Primary Pediatrics' current immunization schedule.  This schedule is subject to change as immunization recommendations change.  Some of the immunizations are available as combination vaccines (e.g. DTaP, Hib, and IPV are combined into the Pentacel vaccine), so there may not be as many actual shots as there are listed immunizations.
  • Birth (in hospital) or 2 week visit (if not done at birth):  Hep B #1
  • 2 months: Hep B #2, DTaP #1, Hib #1, IPV #1, Prevnar 13 #1, Rotateq #1
  • 4 months: DTaP #2, Hib #2, IPV #2, Prevnar 13 #2, Rotateq #2
  • 6 months: Hep B #3, DTaP #3, Hib #3, IPV #3, Prevnar 13 #3, Rotateq #3
  • 9 months: none
  • 12 months: MMR #1, Varicella #1, Prevnar 13 #4
  • 15 months: DTaP #4, Hib #4, Hep A #1
  • 18 months: none
  • 2 years: Hep A #2
  • 4 years: MMR #2, DTaP #5, IPV #4, Varicella #2
  • 2-5 years: Prevnar 13 booster for children younger than 59 months*
  • 11-12 years: Tdap, Menactra #1, Varicella #2**
  • 12-14 years: Gardasil***
  • 15-18 years: Menactra #2

Key:
Hep A = Hepatitis A
Hep B = Hepatitis B
DTaP = Diphtheria, Tetanus, and acellular Pertussis
Hib = Haemophilus influenzae type B
IPV = Polio
Prevnar 13 = Streptococcus pneumoniae, 13 strains
  *Through June 2010, we were giving the original Prevnar vaccine, which protected against 7 strains of pneumococcus. In July 2010, we began using Prevnar 13. Children younger than 5 years who got the original Prevnar and have never received a dose of Prevnar 13 will need to get one dose of Prevnar 13. This will primarily be children younger than 5 who turned 1 before July 2010.
MMR = Measles, Mumps, and Rubella
Varicella = Chickenpox
  **The second dose of varicella is routinely given at age 4. However, for those children that did not get the second dose at age 4, we are giving it at the 11-12 year visit.
Rotateq = Rotavirus (oral vaccine)

Tdap = Tetanus, diphtheria, acellular pertussis (tetanus booster)
Menactra = Meningococcal (meningitis vaccine)
Gardasil = Human papilloma virus (HPV, cervical cancer vaccine)
     ***Gardasil is for girls only and is given as a series of 3 doses over 6 months
