Insurance Information and Financial Policy Primary Pediatrics accepts most major insurance plans.
The following are the plans we currently do not accept: * Priority Partners for new patients except newborns and their siblings * Kaiser Permanente for new or established patients * Amerigroup for new patients (except siblings of already established patients) * United Healthcare through the Maryland medical assistance program for new patients (except siblings of already established patients) * Maryland Physicians Care (MCP) Health Plan
Copayment is due at the time of the visit. We accept cash, credit card (Visa, MasterCard, and Discover only), bank debit card, and personal check. We do not accept cashier checks, company checks, or money orders. There will be a $35 fee for checks mailed to the business office that are returned for insufficient funds.
If you have any doubt about whether we participate with your insurance plan or if you have a question about a specific plan, please call our office or your insurance company to see if we are participating providers before scheduling your appointment.
Forms New Patients Please fill out the following form(s) before your first visit. This will help speed up the registration process and shorten the amount of time you will have to wait while your paperwork is processed.

Patient Registration Form - You may mail or fax the completed Patient Registration Form along with a copy of both sides of the current insurance card to the office where your appointment is scheduled (Laurel, Bowie, or Silver Spring), or you may bring the completed form with you to your appointment. If you mail or fax it, please note which child is being seen and the date and time of the appointment on the fax or cover letter and remember to bring the insurance card with you to your appointment. If you mail the form, please do so at least one week before your appointment date to make sure we receive it in time.

Financial Policy - Please read and sign the Primary Pediatrics Financial Policy and bring the completed form with you to your appointment.

New Patient Record Request Form - It would be helpful if you could complete and send this form to your child's old pediatrician several weeks before your first appointment in our office. This way, there is a greater chance that we will have all of your old records available for review when you arrive for your first appointment.

Current Patient Record Transfer Form - If you are leaving our practice and need a copy of your child's records to take to his or her new doctor, please complete this form and mail, fax, or bring it to the office where your child is usually seen. Keep in mind that it may take up to seven business days to process your request (not including the time it will take in the mail), so you should plan to turn this form in to us a few weeks in advance of your first appointment with your new doctor.

Consent to Complete Medical Forms - If you need a school, sports, or camp physical or medication form completed, please complete this form and mail, fax, or bring it to the office where your child is usually seen. If you bring the school or camp forms to your child's appointment for a well child check or sports physical, we will complete the form(s) during the appointment for no charge. If you bring the form(s) to a sick appointment or your child does not have an appointment when you bring in the form, we will ask you to leave the forms with us. We usually complete them within 3 business days (at busy times of the year it may take a few extra days), and there will be a $10 fee. There is an additional $10 fee for rush or same-day service.
